If you are participating in an activity at the Newport Aquatic Center, you can help speed things up by completing their Release of Liability Form.  To do so, follow these steps and note that these forms need only completed once per calendar year:

1. Download the NAC Release of Liability Form

2. Complete and sign the form

3. Bring the signed form to the event.
